Suicide Mortality Review Cooperative Agreements

In FY24, the Office of Suicide Prevention launched its inaugural year of the Suicide Mortality Review Cooperative Agreements. These cooperative agreements support states, territories, and Tribal entities in establishing, coordinating, and managing Suicide Mortality Review Committees (SMRCs) to identify and characterize suicide deaths and support the capacity to develop and implement data-informed strategies to prevent suicide, including suicide among Veterans.

In FY24, VA provided more than $4M in cooperative agreement funding to 10 states and two territories to support the planning and establishment of new SMRCs and the implementation and expansion of existing SMRCs to focus on Veteran suicide deaths and prevention.

Important dates and events

  • April 15, 2026: SMR optional office hours
  • April 30, 2026: Quarterly progress reports due
  • May 2026: Bi-monthly check in calls scheduled throughout the month

Yes, as stipulated in the notice of funding opportunity, part of VA’s role, via your VA liaison, is to review, provide feedback, and approval as appropriate on materials procured or developed as well as meetings and conference agendas planned for SMR and LMS purposes under this agreement.

Check-in calls will be scheduled with your VA liaison in November 2024, January, March, April, May, July, and September 2025.

Quarterly progress reports will be due January 30, April 30, July 30, December 30, 2025 (final progress report), with the Federal Financial Report due January 30, 2026.

Per 2 CFR 200.308(b) The recipient or subrecipient must report deviations from the approved budget, project or program scope, or objective(s) in accordance with § 200.329. The recipient or subrecipient must request prior approvals from the Federal agency or pass-through entity for budget and program plan revisions. Per 2 CFR 200.308(c) When requesting approval for budget revisions, the recipient or subrecipient must use the same format for budget information that was used in their application, except if the Federal agency has approved an alternative format. Please also pay attention to 2 CFR 200.308(f) revisions that require prior approval. Please send all of your budget/funding related questions and requests to the SMR email (VASuicideMortalityReviewTeam@va.gov).

There will be a maximum of two budget amendments permitted.

We will review requests for a no-cost extension on a case-by-case basis and do not guarantee that such requests will be granted. Please be sure to pay attention to 2 CFR 200.308(f)(10) and the timelines outlined for requesting no cost extensions.
